Implement interventions to improve mobility, strength, balance, gait, and functional independence. Reassess progress, modify treatment plans as needed, and document resident outcomes in charting systems. Collaborate with an interdisciplinary care team to align therapy goals with overall resident care plans. Educate residents and families on home exercise programs, safety strategies, and fall prevention. Ensure compliance with professional standards and facility policies while delivering high-quality care. Requirements Willingness to participate in a travel assignment in Pembroke, NC for 13 weeks. Availability to work the listed shift (08:00–16:30) and maintain 35 guaranteed hours per week. Strong clinical skills in geriatric rehabilitation within a skilled nursing facility setting. Effective communication and documentation skills for multidisciplinary care. Ability to work independently and as part of a team to meet resident goals.
