The LPN is a clinical position responsible for performing medical duties in a healthcare setting, under the direction of a licensed clinician. As an LPN, you will provide clinical support services including direct clinician support, patient care functions, and administrative tasks. You will report to a Supervisor, Center Administration.

This is an onsite role working at our CenterWell clinic located at:

2910 Brownwood Blvd, The Villages, FL 32163

Role Responsibilities:

• Process incoming and outgoing calls/messages.
• Perform patient intake, obtains necessary patient information to file and update patient records.
• Ensure consents are reviewed and completed by clinician.
• Performs pre-encounter prep prior to patients visit, including identifying open care gaps.
• Gather patient data through interview and observation.
• Assist the clinician during medical treatments, examinations, and procedures.
• Prepare patients for examination, performs and documents vital signs, and records patients' health history and medication reconciliation.
• Help arrange laboratory and other ancillary testing.
• Prepares and sanitizes equipment, examination rooms and instruments.
• Manage inventory of medical supplies and equips exam rooms with appropriate supplies.
• Embrace and participates in quality initiatives, goal attainment, and collaborate on clinical process improvement activities.
• Provide routine treatments and administer approved medication per clinician order within scope.
• Document patient data, treatments, procedures, and patient education per organizational policies.
• Assist the clinician by being a liaison on behalf of the patient to schedule appointments; provide patient with scheduling information to obtain testing, and/or clinician consultation.
• Maintain compliance with all policies and procedures.

Required Qualifications

• Active and unrestricted LPN license in the state of Florida.
• 1+ years of experience as an LPN.
• CPR/BLS Healthcare Provider Certification within 90 days of employment.
• Assume responsibility for professional development and continuing education.

Preferred Qualifications

• IV Certification
• Experience with Athena

Annual Requirements:

• Must maintain current license, certification, or registration.
• Must complete required CEUs for licensure/certification
• Must maintain current BLS certification.
• EMR and clinical competencies completed and signed as assigned.

Additional Information:

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$48,900 - $66,200 per year
