LVN / LPN - Ambulatory Care/Clinic/Outpatient

Job Description

Job Summary:
Provide outpatient nursing care for patients in an ambulatory or clinic setting. Support physicians and healthcare teams by performing delegated clinical tasks and ensuring patient comfort and safety.

Responsibilities:

• Assess patient health status and document findings accurately.
• Administer medications and treatments as prescribed by healthcare providers.
• Collect patient specimens and assist with diagnostic procedures.
• Educate patients on treatment plans, medication usage, and health management.
• Maintain comprehensive and up-to-date patient records in the EMR system.
• Adhere to facility policies and infection control procedures to ensure safety.

Qualifications:

• Hold an active LPN license recognized in the state of employment.
• Possess a minimum of 2 years of outpatient LPN experience.
• Demonstrate proficiency in basic patient assessment and clinical skills.
• Have reliable transportation within a 50-mile radius of the facility.
• Exhibit effective communication and teamwork capabilities.
• Be able to work day shifts (7:00 AM - 3:30 PM) without call, weekends, or holidays.
