At Select Home Health & Hospice, we believe in putting patients first: “Select the best for your health!”Job SummarySelect Home Health is seeking skilled and compassionate Registered Nurses to provide in-home nursing care in West Lafayette, Indiana and surrounding areas. This role focuses on delivering high-quality, personalized care while promoting patient independence and overall well-being.Duties/Responsibilities:Perform patient assessments and ongoing evaluations.Implement physician-ordered plans of care and complete required documentation (OASIS).Provide in-home nursing care for multiple patients per day.Educate patients and caregivers on medications, disease processes, and in-home care.Coordinate patient services and supervise LPNs/HHAs as needed.Communicate changes in patient condition to physicians and care teams.Maintain compliance with regulations and agency policies.Physical RequirementsAbility to sit, stand, and drive for extended periods while providing in-home care.Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.Regular use of a tablet for documentation, communication, and training.BenefitsComprehensive healthcare coverage, including medical, dental, vision, FSA/HSA, life, and disability insurance.Paid Time Off (PTO) for vacation, personal, and sick days.401(k) retirement plan with company match after 1 year of employment.Disclaimer:This description reflects the general nature and level of work for the Registered Nurse Position.
