Description: The RN implements professional nursing practice centered on the patient as a whole person. The practice focuses on the creation of a caring environment aimed at healing and wellness. Nursing practices are based on evidence and current research, as well as departmental policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across disciplines. The RN coordinates, ensures, and provides direct patient care activities through various responsibilities. These include provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also a key responsibility. The RN provides patient and family-centered education, care coordination, and individualized treatment planning. A comprehensive assessment is performed and documented in accordance with the RN's scope of practice and departmental policies. Patient culture, age, and developmental levels are considered during assessments. The RN identifies patient risk factors and presenting complaints and collects both objective and subjective data. Developing a problem list and prioritizing patient needs based on input from the inter-professional team is essential. The RN designs actions and interventions relevant to the patient's culture and age. Determining desired outcomes in coordination with the patient and family is also part of the responsibilities. Patient care therapies and procedures are fully implemented while adhering to established policies and professional guidelines. The RN explains procedures and treatments to the patient and family and initiates actions to alleviate stress and discomfort. Recognizing and responding to changes in patient condition is vital. Conducting and documenting ongoing assessments and responses to procedures is required according to hospital policy. The RN delegates tasks appropriately and oversees team members in the performance of patient care activities. Monitoring the accuracy and completeness of delegated patient care is part of ensuring the implementation of the plan of care. The RN identifies and addresses educational needs of patients based on prioritized problems and levels of comprehension. Facilitating the patient discharge process by executing discharge orders timely is necessary. Providing input into patient/nurse assignments considering available staffing, unit activity, and patient acuity is also important. The RN demonstrates active participation in unit-based activities to support organizational values. Promoting a positive group dynamic and healing environment through words and actions is essential. Ensuring nursing excellence in line with accepted standards and evidence-based practices is pursued actively. The RN participates in department and/or unit quality improvement activities and metrics such as patient satisfaction and falls. A holistic perspective towards patient care is embraced.
