Registered Nurse (RN) - Case Management

Case Management – Acute Care

Every other weekend required. Must commit to working 3 of 5 major holidays.

Minimum 2 years recent acute hospital case management experience. Acute care case management experience must be current within the last year. Experience with HMOs/IPAs and utilization/benefits processes. Proficient in Epic.

Manage a caseload of up to 25 patients per day, coordinating across interdisciplinary teams to facilitate discharges. Conduct needs assessments and develop discharge plans that address complex patient needs. Oversee the authorization, denial appeals, and benefits eligibility for patients under care. Ensure compliance with CMS/JCAHO core measures, working with advance directives and protective services as needed. Familiarity with processes related to worker's compensation is beneficial.
