Hours of Work: flexible, up to 8 hours/week Rate: to be negotiated, based on qualifications Start Date: as soon as possible Posting Date: August 12, 2026 Competition Closing Date: until suitable candidate is found Job Description The Clinical Care Coordinator (CCC) role combines quality improvement, panel management, program planning, and team development activities within a clinic environment to support the participation of the clinic in the Chinook Primary Care Network. Prairie Treatment Clinic (PTC) offers opioid agonist therapy (OAT), including methadone, suboxone and sublocade for individuals with opioid use disorder (OUD). The Clinical Care Coordinator working in PTC will provide a patient-centered approach to ensure patients receive safe, supportive and confidential care while being treated for their OUD. PTC may be the first point of contact for some individuals who do not presently have a primary care provider. As patients receive treatment and support for their OUD, primary care may become more essential for their ongoing recovery. It will then be appropriate to deliver some primary care services and facilitate their connection to other primary care providers. While practicing within their scope of nursing practice, the CCC will work with other team members to facilitate quality improvement and best practice initiatives to provide excellent care to patients and indirectly to families and communities. Position Qualifications RN degree preferred. Current licensure with provincial Nursing College, in good standing is required. LPN with advanced practice experience may be considered. Minimum 3 years’ experience in a primary care, community health, ambulatory care, and / or addictions and mental health setting. Preference will be given to candidates with experience working to full scope of practice. Experience working with Addictions and Concurrent Disorders an asset Knowledge or willingness to develop knowledge and skills in opioid use disorder and treatment Strong understanding of the Social Determinants of Health Current Certification in Sublocade administration, or willingness to become certified Demonstrates excellent clinical skills & knowledge Excellent written and verbal communication skills Strong leadership, facilitation, and organizational skills Experience working with a variety of service providers in a collaborative setting Ability to maintain patient privacy & confidentiality Knowledge and skills in quality improvement and measurement an asset Knowledge and skills in health education for patients and their families Proficiency or a willingness to develop proficiency in use of computers including MS Office, E-mail, MS Excel, PowerPoint, and electronic medical record software Knowledge and skills or a willingness to develop knowledge and skills in program planning and evaluation Knowledge and skills or a willingness to develop knowledge and skills in management of chronic diseases Current certification in Basic Cardiac Life Support preferred For further information about the Chinook Primary Care Network: #J-18808-Ljbffr Chinook Primary Care Network Ltd.
