Job Description
Physical Therapist

We are seeking a dedicated Physical Therapist to provide comprehensive evaluations and treatments that enhance patients' functional abilities. You will oversee and guide the work of Physical Therapist Assistants and other designated staff members in delivering high-quality care.

Key Essential Functions and Responsibilities

Conduct general physical therapy evaluations and treatments for a diverse patient population across various age groups and diagnoses.
Perform assessments, identify therapy diagnoses, and formulate individualized treatment plans with specific goals.
Implement, supervise, and modify therapy plans based on patient assessments to optimize outcomes.
Review clinical records and case histories and participate in care conferences and meetings to update the healthcare team on patient status.
Collaborate with team members to ensure appropriate medical equipment is in place and care plans are established prior to patient discharge.
Maintain accurate documentation of patient encounters, including findings, treatment plans, recommendations, and prognoses in compliance with relevant policies.
Provide timely services in accordance with regulatory requirements while working with in-patient and out-patient populations.
Guide and direct support staff, students, or interns as needed.
Foster a healthcare partnership between providers, patients, and their families.
Develop, implement, and conduct educational programs for patients and families tailored to their needs, ensuring understanding of prescribed home exercise programs.
Assist in administrative tasks and operations related to therapy services as appropriate.
Utilize information technology effectively for obtaining patient information and maintaining communication.
Contribute to the acquisition and fitting of durable medical equipment based on patient needs.
Ensure supply stocks and clinical equipment are maintained at acceptable levels.
Participate in maintaining the cleanliness and organization of clinical spaces.
Adhere to all relevant practice standards and regulations, focused on quality assurance and patient confidentiality.
Perform additional duties as assigned.

Qualifications

Master's or Doctorate degree in Physical Therapy from an accredited program, or equivalent graduate-level education with proven experience.
Licensed as a Physical Therapist.
Current Basic Life Support (BLS) certification.
Valid Driver's License.
Clinical Competency required within three months of hire and recertified every three years.
High Risk Competency required within three months of hire and recertified every three years.

Experience and Skills

Broad-based clinical experience in multiple settings preferred.
Knowledge of musculoskeletal physiology, neuro system assessment, and general medical conditions.
Proficient in assessment techniques, treatment planning, and therapy for patients with various diagnoses.
Effective communication skills for interaction with patients, families, and healthcare professionals.
Ability to utilize and fit adaptive devices for patient rehabilitation.
Strong proficiency with information technology and various software applications.
Capable of showcasing sound judgment and applying critical thinking in patient care and workplace interactions.
