Family Support Specialist - In-Home & Community

One Hope United

One Hope United seeks a Family Support Facilitator in Texas to provide direct in-home and community services to moderate and high-risk families, using Solutions Based Casework and Family Team Conferencing as primary methods. You will attend internal and external meetings, review intake information, coordinate transportation, document thoroughly, and build partnerships with community providers, while upholding OHU policies and pursuing ongoing trainings. #J-18808-Ljbffr One Hope United
