Registered Nurse Telephonic Care Manager

Working remotely in a full-time capacity, the Registered Nurse Telephonic Care Manager will manage patient transitions of care following significant healthcare events, conducting comprehensive assessments, developing individualized care plans, and coordinating resources to ensure optimal patient outcomes. Key responsibilities Conduct comprehensive assessments to identify patient needs and develop individualized care plans Coordinate transitions of care across various healthcare settings and arrange necessary follow-up services Connect patients with community resources and advocate for their needs throughout the care transition process Required qualifications Active Registered Nurse (RN) licensure Bachelor's Degree in Nursing preferred Five (5) years of experience in care management or related clinical experience Certified Case Manager (CCM) or equivalent certification preferred Knowledge of post-acute care services and healthcare delivery systems
