Responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care. \n \n CONNECTING TO MISSION: \n \n \nAll work is performed within a resident centered frame work in collaboration with other healthcare professionals, residents and their families. Functions as a role model for staff. Recognizes the value of each person as an individual. Supports and Promotes Resident/Patient strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based.\n \n \nRECRUITMENT REQUIREMENTS\n \n \nCurrent LPN license\n \n \nPrevious experience preferred\n \n \nDemonstrates position communication skills\n \n \nAbility to relate to staff in a professional manner\n \n \nMust be flexible with scheduling\n \n \nComputer experience preferred\n \n \nDemonstrates sensitivity to residents emotional, social, mental health and spiritual needs\n \n \nPrevious experience in a rehab setting preferred.\n \n \nMust be able to interact well with physicians and other health care professionals\n \n \nAble to exercise sound professional judgment\n \n \nAble to respond to emergency\n \n LICENSE AND REGULATORY REQUIREMENTS \n \n \nCurrent LPN License\n \n EOE \n \n \nEQUAL OPPORTUNITY\n \n \nRedeemer Health is an equal opportunity employer.
