Role Description The Case Manager is a full‑time, on‑site position based in Moapa, NV, responsible for coordinating and delivering supportive services to individuals and families within the community. This role includes assessing client needs, developing service plans, connecting clients to appropriate resources, and monitoring progress toward identified goals. The Case Manager will maintain accurate and timely documentation, coordinate with Tribal departments and external agencies, and advocate for clients to ensure access to health, social, and educational services. Daily tasks also involve conducting follow‑up appointments, participating in multidisciplinary meetings, and providing crisis intervention and referrals when necessary. The Case Manager is expected to uphold confidentiality, demonstrate cultural sensitivity, and contribute to continuous improvement of Tribal programs and services. Qualifications Candidates should possess strong case management and client service skills, including assessment, care planning, coordination of services, and advocacy. Candidates should possess effective communication skills, including clear verbal and written communication, active listening, and the ability to build trust with diverse clients and partners. Candidates should possess organizational and documentation skills, such as record‑keeping, time management, prioritization of caseloads, and attention to detail. Candidates should possess collaborative and teamwork skills, including the ability to work with Tribal departments, community organizations, and external agencies to support client outcomes. Candidates should possess basic computer and administrative skills, including proficiency with e‑mail, word processing, case management software, and data entry. Relevant education in social work, human services, counseling, psychology, or a related field is beneficial; a bachelor’s degree or equivalent experience is preferred. Experience working with Indigenous communities, Tribal governments, or in rural or underserved settings is beneficial, as is knowledge of culturally responsive practices. Ability to maintain confidentiality, exercise sound judgment, and demonstrate reliability, professionalism, and respect for Tribal customs and values is essential. #J-18808-Ljbffr
