About the Role:
Join MedPOINT Management as a CCS Nurse Case Manager, where you will play a vital role in enhancing patient care and outcomes. This position offers an exciting opportunity to work within a dynamic team in Sherman Oaks, CA, dedicated to providing exceptional healthcare management services.
Responsibilities:
Conduct comprehensive assessments to develop and implement individualized care plans for patients.
Coordinate with healthcare providers, patients, and families to ensure effective communication and care continuity.
Monitor patient progress and adjust care plans as needed to optimize health outcomes.
Facilitate access to necessary resources and services, including medical, social, and community support.
Document all interactions and maintain accurate records in compliance with regulatory standards.
Provide education and support to patients and families regarding treatment options and healthcare navigation.
Participate in interdisciplinary team meetings to discuss patient cases and share insights.
Stay updated on industry trends and best practices in case management and patient care.
Requirements:
Active RN license in the state of California; CCS certification preferred.
Minimum of 3 years of nursing experience, with a focus on case management or related fields.
Strong critical thinking and problem-solving skills to assess complex patient needs.
Excellent communication and interpersonal skills for effective collaboration.
Proficiency in electronic health records (EHR) and case management software.
Ability to work independently and manage multiple cases efficiently.
Compassionate demeanor with a patient-centered approach to care.
Knowledge of community resources and healthcare systems.
