Job Title: Licensed Practical Nurse (LPN) – Long Term Care (LTC)

Location: Thorp, WI
Job Type: Contract / Full-Time
Schedule: Variable Day & Evening Shifts | 8-hour shifts
Guaranteed Hours: 40 hours per week

Compensation:

Local: From $35/hour
Travel: From $1,600/week

Position Summary

We are seeking a reliable and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and rehabilitation setting .

This role involves delivering compassionate, resident-centered care , supporting clinical operations, and collaborating with interdisciplinary teams to ensure optimal patient outcomes . The ideal candidate is flexible and comfortable working day and evening shifts .

Key Responsibilities Patient Care & Treatment

Administer medications and treatments as prescribed
Provide direct nursing care while ensuring resident safety and comfort
Monitor resident conditions and report changes to the care team

Clinical Support & Documentation

Assist with:
- Resident assessments
- Care planning
- Clinical documentation
Maintain accurate and timely patient records

Team Collaboration

Work closely with:
- Certified Nursing Assistants (CNAs)
- Nursing staff
- Interdisciplinary care teams
Support coordinated and efficient care delivery

Requirements

Active Licensed Practical Nurse (LPN) license (required)
Experience in:
- Long-Term Care (LTC) or
- Skilled Nursing Facility (SNF) (preferred)
Ability to work:
- Rotating day and evening shifts
- Every other weekend
Flexibility to work 5 shifts per week as needed

Overtime opportunities available
Flexible scheduling options, including:
- Two 16-hour shifts + one 8-hour shift (where applicable)
