Position Type: Full Time Service Worker – Nashville, TN, US. Schedule: 2nd Shift – 4:00 PM to 12:00 AM. Salary: $16.50 to $18.00 per hour (plus shift differentials). Sign-on Bonus: $500 for full-time employment. Two weeks paid training, 9 AM to 5 PM, Monday to Friday. Responsibilities Represent OAC in a positive and professional manner at community meetings, events, and social functions. Provide day‑to‑day organization and implementation of activities that promote an organized and positive environment. Act independently to make decisions regarding best practices, quality assurance, risk management, and problem‑solving, and timely response to and documentation of serious injuries. Coach, teach, and mentor individuals in all aspects of home management and personal independence, actively promoting individual rights and self‑advocacy. Engage clients in ongoing, meaningful active treatment and the individual support plans (ISP’s) for the clients served in the ID/DD program; implement programs and delivery of services according to the ISP and CMS regulations. Advocate for each client’s specific needs and personal welfare in the home and community, and ensure follow‑up when concerns are identified. Assist with coordinating, scheduling and/or transporting clients to appointments and recreational activities; ensure required documents, files and reports are prepared, reviewed, approved and processed timely. Provide accurate documentation of client participation and progress in all training objectives/programs to teach independent living skills; ensure day‑to‑day assigned tasks are completed. Communicate and actively participate in the inter‑disciplinary process to include individual support plans, behavior management, incident reports, etc. Interact with client’s family members/conservators, maintaining positive relationships internally and externally. Attend all required trainings/meetings and remain current on certifications. Provide support and services to all clients across the local operation when necessary. Transport clients to medical appointments, Enrichment Center, and client outings as assigned. Maintain work schedules; dependability is required; monitor and manage own time off and document time worked accurately. Carry out responsibilities in a professional, timely, effective, and efficient manner, including other duties as assigned. Participate in additional committees or volunteer activities as assigned. Qualifications High school graduate and/or GED preferred. Valid driver’s license required. CPR certification within 30 days of hire required. Certified in Professional Crisis Management (PCM) within 60 days of hire required. Ability to lift repetitively a minimum of 50 pounds; tasks may include lifting, repositioning, turning, bending, squatting, etc. Good oral, written, and interpersonal skills to effectively interact with others. Ability to maintain confidentiality. Ability to follow procedures, instructions, laws and policies. Ability to project a positive and professional image on behalf of the organization and be an effective team member. Ability to prioritize and manage time effectively.
