Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in an outpatient setting. The position involves evaluating, treating, and planning patient care for adult and geriatric populations with orthopedic, neurological, and functional mobility diagnoses. The role requires collaboration with a team to meet regulatory standards and deliver quality patient care, with a focus on resilience and adaptability in a dynamic environment.

Job Details:

• Shift: M-F, 8:00 AM - 5:30 PM
• Schedule: 8 hours per day, minimum of 40 hours per week
• Start Date: 10/05/2026
• End Date: 01/02/2027
• Required Qualifications: Bachelor's degree, State licensure in Wisconsin, Certification in American Heart Association Basic Life Support (AHA BLS), National Provider Identifier (NPI) (or the process to obtain one started), minimum of 2 years of experience in home health, training with adult/geriatric populations with orthopedic, neurological, and functional mobility diagnoses, and knowledge of Physical Therapy standards acquired through a Master's degree in Physical Therapy.
• Preferred Qualifications: Home health experience, strong documentation skills using NetSmart electronic medical record, familiarity with GPS and communication technology, resilient, adaptable, social, and capable of managing change.
• Additional Information: Occasional travel between branches may be required, with mileage reimbursed at government-approved rates. The position may include on-call hours for low census periods, which count toward unbillable hours. During flu season, unvaccinated travelers must wear masks at all times, in accordance with organizational policy.
