Key Responsibilities- Maintain an excellent work-life balance
- Work from 8 AM to 5 PM, supported by an RN triage call center
- Focus on Senior Primary Care, specifically Medicare Advantage patients
- Engage in meaningful interactions with patients
- Experience with athenahealth EMR and HEDIS measures is preferred
- Ability to thrive in a collaborative environment with strong nursing support
