Role Description The Licensed Social Worker will provide direct client services, including assessments, case management, and referrals to appropriate community resources., in support of the organization’s economic and community development programs. Day-to-day responsibilities include meeting with individuals and families, developing service plans, coordinating with local agencies and service providers, and documenting client interactions in accordance with regulatory and organizational standards. This is a full-time, on-site role located in Pontotoc, MS. Qualifications Licensed Social Worker (LSW) in the state of Mississippi, Demonstrated skills in client assessment, case management, and referral coordination within community or human services settings. Strong interpersonal, communication, and relationship-building skills to work effectively with clients, families, community partners, and multidisciplinary teams. Ability to maintain accurate documentation, comply with regulatory requirements, and use basic computer applications and case management systems. Capacity to work on-site full time in Pontotoc, MS, with reliability and professional judgment. LSW Case Management for Home and Community Based Services/Elderly and Disabled LSW and RN work in a partnership, making home visits to patients to coordinate and monitor at-home services. A work vehicle is provided. Schedule is M-F 8:00 AM- 4:30 PM OR 4 day work schedule with Friday or Mondays off. #J-18808-Ljbffr
