Medical Doctor

Under the direction of the Regional Medical Director, the Primary Care Provider (PCP) provides primary care services for participants of the assigned PACE Center. This includes assessment, development of care plans in conjunction with the interdisciplinary team (IDT), provision of direct medical care and treatment, and evaluation of the effectiveness of care. Responsibilities Participates as a member of the IDT and conducts initial, semi-annual, and unscheduled reassessments as required. Develop and implement care plans for participants in conjunction with the IDT. Attends morning meeting and reports changes in participants’ baseline to appropriate staff daily. Obtains a complete health history and records the findings in a systematic and organized manner. Completes charting and paperwork in a timely manner. Performs functional status evaluations and/or mental status examinations using standard procedures. Triages status changes in participants and facilitates appropriate management of problem(s) identified. Provides preventative health care and health promotion for participants. Treats and manages acute and chronic illnesses. Regulates and adjusts medications as needed. Facilitates medical specialist consultation as needed. Educates participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff. Understands fiscal issues and is appropriately involved in meeting budget targets. Consistently meets or exceeds company's targets for productivity. Continually seeks better ways for delivering services and communicating with participants. Consistently meets or exceeds company's customer service targets. Demonstrates respect for and promotes participant rights including dignity, self- determination, access to care, confidentiality, and independence. Understands the importance of community involvement and participates as appropriate in activities that link PACE to its communities. Effectively collaborates with staff peers and contractors to meet Kinship Health goals and to further success. Demonstrates proficiency in delivering age-specific care. Consistently meets or exceeds the company's quality assessment and performance improvement targets. Attends and participates in staff meetings, in-services, projects, and committees as assigned. Adheres to and support the center’s practices, procedures, and policies including assigned break times and attendance. Accepts assigned duties in a cooperative manner; and performs all other related duties as assigned. Flexible in the schedule of hours worked. On-call hours are required. May require use of personal vehicle. Qualifications Graduate of an accredited school of medicine. Board certified in internal medicine or family practice. Board certification granted by the American Boards of Internal Medicine and Family Practice for Added Qualification in Geriatric Medicine preferred. Licensed by the State Medicine Board to practice medicine. Minimum of two (2) years of experience in caring for seniors. Minimum of two (2) years of documented experience working with a frail or elderly population in a long-term care setting preferred. Medically cleared for communicable diseases and has all immunizations up to date before engaging in direct participant contact. TB screening and successful completion of a health examination by a provider is required for employment. CPR certification required. Valid and up-to-date DEA certificate is required. Basic Life Support certified by the American Heart Association or American Red Cross required. Minimum two (2) years' experience working as an Adult, Family, or Geriatric NP, including one (1) year of experience working with frail elderly, preferably in a long-term care or primary medical care setting. Valid driver’s license. Required Skills Medical knowledge of physical, mental, and social needs of elderly adults. Knowledge and ability to maintain a consistent level of productivity. Understanding of detailed requirements in action plans and implementation. Knowledge required to commission and conduct data analysis for decision-making. Excellent interpersonal skills that result in team building, successful negotiation, conflict resolution, and effective handling of complex interpersonal situations. Leadership skills to work effectively within an interdisciplinary team setting. Ability to inspire confidence and open communication necessary for a learning environment. Ability to maintain a positive attitude and balance in relationships with others. Effective social interaction with physicians, management, board members, and business and community contacts. Strong organizational and supervisory skills. Ability to complete duties within an agreed-upon timeframe and adapt appropriately to change of priorities and workload and to adjust personal schedule, if required. Sound written and verbal communication skills to convey information effectively. Able to perform responsibilities with frequent interruptions and competing priorities. Work with moderate exposure to excessive noise, dust, and temperature. Utilization of personal computers for clinical/financial database systems. Proven track record of strategic leadership, program expansion, and provider relations in a complex healthcare environment.
