Licensed Vocational Nurse (LVN)

As a Licensed Vocational Nurse (LVN), you will assess, plan, and implement the nursing care of residents within a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing. Your responsibilities will include administering medications while adhering to medication safety protocols, ensuring continuity of care between shifts, and supervising the care provided by Certified Nursing Assistants (CNAs) and support staff. You will also participate in resident and family education and maintain a strong focus on resident advocacy in alignment with the facility's mission.

Roles & Responsibilities

• Assess resident conditions and nursing needs, prescribing appropriate nursing actions.
• Effectively assess and manage pain, utilizing appropriate pain management techniques while educating residents and their families.
• Document changes in residents' conditions and nursing care in progress notes and care plans.
• Perform waived testing in accordance with Clinical Laboratory and unit policies.
• Interpret results of waived tests and take appropriate action based on the findings.
• Administer medications, ensuring compliance with the eight medication rights to minimize potential errors.
• Meet current documentation standards and policies.
• Notify physicians of changes in residents' conditions and follow up until appropriate actions are executed.
• Complete physician's orders timely.
• Provide comprehensive reports during shift changes through walking rounds and appropriate documentation.
• Follow up on resident and family concerns as needed.
• Implement resident care plans based on interdisciplinary team assessments and established goals.
• Treat residents and their families with respect and dignity, addressing their psychosocial, cultural, ethnic, and spiritual needs.
• Supervise team members to ensure the care plan is accomplished effectively.
• Demonstrate competency in technical and manual skills according to LVN scope of practice.
• Function as a resident advocate by protecting their rights.
• Utilize creativity and problem-solving skills in unique situations.
• Interact with residents, families, staff, and management in a friendly and knowledgeable manner.
• Involve residents and family members in care planning, treatment, and discharge processes.
• Provide leadership to CNAs and delegate responsibilities to enhance unit excellence.
• Assist in training new personnel and participate in performance evaluations as needed.
• Adhere to infection control procedures and supervise staff compliance in infection control practices.
• Remain calm and handle emergencies or stressful situations effectively, instilling confidence in staff and residents alike.
• Consistently follow facility policies and procedures.
• Exhibit a genuine interest in and appreciation for the elderly.
• Maintain motivation and independence while organizing workload efficiently.

Qualifications & Skills

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse.
• Current BLS certification.
• Ability to read and communicate effectively in English; additional languages are preferred.
• Basic computer knowledge.
• Electronic Health Record experience is preferred.
