LPN (Licensed Practical Nurse) - Evening Shift

Provide direct patient care, including administering medications, performing vital signs, and monitoring patient conditions
Develop and implement individualized care plans that address the unique needs and goals of each patient
Collaborate with the interdisciplinary care team to ensure comprehensive care and coordination of services
Document patient care and progress in accordance with facility policies and regulations
Participate in quality improvement initiatives and contribute to the development of best practices
