As a Home Care Specialist, you will operate independently or with minimal supervision, bringing the skills of a licensed physical therapist directly to those in need. Our work environment encourages open communication and ongoing education, fostering a culture of teamwork and collaboration in delivering top-notch orthopedic care.Key ResponsibilitiesConduct thorough evaluations of patients' physical capabilities and create effective treatment plans tailored to individual needs.Administer therapeutic treatments including modalities, manual techniques, and customized exercise programs aimed at improving mobility and strength.Guide patients through functional activities such as bed mobility, transfers, gait training, and balance exercises to ensure safety and promote independence.Utilize various rehabilitation equipment, such as walkers and strength-training devices, to enhance therapy effectiveness.Document patient progress and communicate findings to supervising physical therapists, ensuring continuity of care.Educate patients and their families on therapeutic exercises and continued care strategies to support rehabilitation post-treatment.Mentor and train junior staff on technical skills and patient care best practices as needed.Patient Population / Client FocusYour patient population will primarily consist of individuals recovering from orthopedic surgeries or injuries. This includes but is not limited to patients with joint replacements, fractures, and chronic pain conditions. You will play a crucial role in helping them regain mobility, strength, and overall function, positively impacting their quality of life.QualificationsBachelor's Degree in Physical Therapy from a CAPTE-accredited program.Active California Physical Therapist Assistant License.Minimum of 2 years of relevant experience in orthopedic physical therapy.Exceptional ability to communicate effectively with patients, family members, and team members.Strong clinical reasoning and decision-making skills to support effective patient-centered care.Preferred ExperienceExperience in home health therapy settings.Familiarity with common orthopedic assessment and treatment protocols.Proficient in using computerized documentation systems for patient records.Compensation and BenefitsThis position offers a competitive salary ranging from $50,000 to $70,000 per year, commensurate with experience.
