Physical Therapist - Home Health

Location: Princeton North, NJ
Schedule: 40 hours weekly, Monday to Friday, 8am to 430pm

Position Overview

We are seeking a compassionate and experienced Physical Therapist (PT) with at least 1 year of clinical experience to provide high-quality home health services to patients throughout Pennsylvania. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions in the comfort of patients' homes to improve mobility, strength, independence, and overall quality of life.

Responsibilities

• Conduct comprehensive patient evaluations and assessments in the home setting
• Develop and implement individualized treatment plans based on patient needs and physician orders
• Provide therapeutic exercises, gait training, balance training, transfer training, and mobility education
• Monitor patient progress and modify treatment plans as needed
• Educate patients and family members on exercises, safety techniques, and home care plans
• Complete accurate and timely clinical documentation in accordance with agency and regulatory standards
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care
• Ensure compliance with all state, federal, and agency policies and procedures
• Participate in case conferences and interdisciplinary team meetings as required

Requirements

• Active Pennsylvania Physical Therapist license
• Minimum of 1 year of Physical Therapy experience required
• Home health experience preferred
• Current BLS certification preferred
• Strong clinical assessment and communication skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid driver's license required
• Experience with EMR/documentation systems preferred

Preferred Qualifications

• Experience working with geriatric and rehabilitation patients
• Knowledge of OASIS documentation
• Strong patient education and care coordination skills
