- Conduct assessments in both inpatient and outpatient contexts while crafting personalized treatment paths for each patient
- Deliver targeted physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
- Work alongside nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
- Guide patients and their families through therapeutic objectives, discharge preparations, and home exercise routines
- Keep detailed documentation of evaluations, treatments, and patient progress in compliance with facility protocols and regulations
- Participate actively in care conferences and interdisciplinary meetings to ensure seamless care coordination
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (neurological, geriatric) are beneficial but not mandatory
- Prior experience in physical therapy is highly preferred
- Expertise in managing patients with complex medical backgrounds and mobility challenges
- Exceptional communication and collaboration abilities
- Familiarity with digital documentation and clinical tracking systems
