As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and rehabilitation services to our patients and residents.

Responsibilities:

• Provide patient care and rehabilitation services under the supervision of a RN or physician, including assessing patient needs, developing care plans, and implementing treatment programs

• Collaborate with the interdisciplinary team to ensure comprehensive care and achieve optimal patient outcomes

• Administer medications, treatments, and injections as prescribed by physicians

• Monitor patient vital signs and report any concerns or changes to the RN or physician

• Maintain accurate and detailed patient records, including progress notes and treatment plans

• Participate in the development and implementation of individual patient care plans

• Continuously assess and evaluate patient care needs and make adjustments as necessary
