Job Summary The College of Nursing (CON) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services (DMS) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based (HCB) waiver assessment tool for the frail, elderly, and physically disabled population. The position will identify, develop, and present information educating professional staff, families, and individuals seeking HCB services while also making sound independent determinations regarding the level of care and services needed for this vulnerable group. The majority of in‑home assessments will occur in Jefferson County, but you may be asked to complete assessments in surrounding counties. Responsibilities Perform in‑home assessments using the HCB waiver assessment tool. Identify and document care needs and level of services required. Educate professional staff, families, and clients about HCB services. Make independent determinations regarding level of care and services. Travel throughout the state on a regular basis; be available for overnight out‑of‑state trips when needed. Skills, Knowledge, and Abilities Knowledge of physical and psychological development; excellent analytical and decision‑making skills; ability to communicate effectively and interact with physically disabled and elderly individuals. Preferred Education/Experience Experience working with elderly and physically disabled patients; valid driver’s license. Benefits No overtime; full 40‑hour workweek.
