InnerSourced Solutions Inc., has an immediate opening for a part-time Youth Care Coordinator with full-time growth opportunity.
Position Overview: Work with youth (ages 6-22) and their caregivers to identify needs and goals that will strengthen the family and the youth's future. Areas of focus include education, mental health, physical health, economic needs, and positive social development. While face-to-face visits occur twice monthly, you will actively work on behalf of each client and family weekly through research, resource coordination, collateral contacts, and system navigation.
Hours & Compensation:
Current: Part-time, 20-25 hours per week at $25/hour
Full-time opportunity: Grows to 40 hours per week as you successfully market the program and increase caseload from to capacity of 23 clients
Your ability to build community relationships and increase enrollment directly determines your transition to full-time hours.
Key Responsibilities:
- Providing in-person case management services to children and adolescents based on authorized level of care, including a minimum of two face-to-face visits per month with ongoing weekly work on behalf of clients and families as needed
- Conducting twice-monthly face-to face visits in homes, schools, or community settings
- Assessing needs, researching and collaborating/connecting clients/families with resources, services and support systems
- Assisting clients/families to navigate healthcare and education systems and understand their options
- Connecting participants to programs and services based on identified needs
- Marketing the Youth Care Coordination through outreach to schools, pediatrician offices, and other referral sources
- Completing required documentation and progress notes within a timely manner
- Participate in weekly supervision
Work Environment: This is an office-based position with community visits (home visits, school meetings, etc.) with rigorous oversight, productivity tracking, and documentation requirements.
What You Should Know About This Role:
- Active case management is ongoing—while clients are seen face-to-face twice monthly, you'll be working on their behalf weekly through research, coordination with service providers, advocacy, follow-up calls, and documentation
- This position requires strong follow-through and organizational skills
- You will have weekly supervision that includes review of specific client interactions, progress notes, and productivity metrics
- All client visits are verified and billable hours are tracked closely
- This role requires consistent, reliable performance—we do not bill for services without complete and timely documentation
- Work varies in pace; you must be self-directed enough to stay productive during slower periods and organized enough to manage multiple priorities during busier times
Qualifications:
- Bachelor's degree in human services or related field REQUIRED and and 1 year of mental health experience 1 year of mental health experience, including mental health peer support or a Bachelor's degree in a field other than mental health and 2 years of verifiable mental health experience, including mental health peer support.
- Reliable transportation and valid driver's license (mileage reimbursement provided)
- Strong organizational and follow-through skills
- Ability to work independently with accountability
- Experience with or genuine interest in working with youth and families
- Proficient with technology, as we use an electronic medical records system
