Social Worker - MSW (Inpatient)

Description A Social Worker - MSW (Inpatient) is a crucial member of the healthcare team, providing comprehensive support and services to patients and their families. They are responsible for conducting assessments, supporting patient‑centered care plans, facilitating resources, monitoring progress, advocating for patients, actively listening to their concerns, respecting individuality, building trust, performing risk segmentation, carrying out delegated work, coordinating care, and communicating with payers. Responsibilities Assessments: Thoroughly assess patients' mental, emotional, and social well‑being to evaluate their needs and identify barriers. Patient‑Centered Care Plans: Collaborate with providers, patients, families, and the healthcare team to develop individualized care plans aligned with goals and preferences. Progress Monitoring & Evaluation: Continuously monitor progress, measure outcomes, and adjust interventions as needed. Advocacy: Advocate for patients' needs within the healthcare system and community. Active Listening: Engage in active listening and provide a safe, non‑judgmental space. Respect for Individuality: Tailor interventions to each patient's unique values, beliefs, and cultural background. Building Trust: Establish trust‑based relationships with patients and families through empathy and professionalism. Risk Segmentation: Identify patients requiring additional support based on risk factors. Tasks or Interventions: Carry out specific tasks or interventions delegated by the healthcare team. Delegated Work: Coordinate referrals, follow‑up assessments, and ongoing support. Care Coordination: Coordinate care across providers and settings for seamless transitions. Payer Communication: Communicate with payers to obtain authorizations and reimbursements. Other Information Education Requirements Master's Degree in Social Work. Licensure/Certification Requirements None required. Professional Experience Requirements Two (2) years health care experience strongly preferred. Knowledge, Skills, and Abilities Requirements Knowledge of mental health conditions, diagnostic criteria, and evidence‑based treatment modalities. Strong assessment and diagnostic skills. Excellent communication and interpersonal skills. Ability to collaborate effectively as part of an interdisciplinary team. Empathy, compassion, and cultural sensitivity. Strong organizational and time‑management skills. Ability to maintain confidentiality and adhere to ethical guidelines.
