Physical Therapist – Home Health (Orthopedic Focus)
Greater Nashua, NH (serving surrounding communities)
$10,000 Sign‑On Bonus
$40/hr – $57/hr (based on experience)

Our client, a long‑established and highly respected home health organization, is seeking a Home Health PT with an orthopedic focus to support patients across the Greater Nashua region.

What You’ll Do
- Develop and implement individualized therapy plans to restore function, strength, and mobility
- Evaluate patient progress and adjust treatment plans as needed
- Complete patient admissions and evaluate clinical needs
- Educate patients, families, and caregivers on home exercise programs
- Ensure continuity of care and proper discharge planning
What We’re Looking For
Required:
- Licensed Physical Therapist (NH)
- Bachelor’s degree in Physical Therapy
- 2+ years of clinical experience (hospital or outpatient)
- CPR certification
- Valid driver’s license + reliable transportation
- Strong independent clinical judgment
Preferred:
- Master’s or Doctorate (PT)
- Home health experience
- Orthopedic background
