This is a field-based home health role primarily serving patients in Davis and surrounding communities, with documentation completed from the field. Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred. Job Description Evaluate patients in their homes and assess mobility, strength, balance, coordination, pain, and functional limitations. Develop individualized physical therapy treatment plans based on each patient's goals and clinical needs. Provide therapeutic exercises and interventions designed to improve mobility, strength, function, and independence. Educate patients and caregivers on exercises, safety precautions, fall prevention, and self-care strategies. Monitor patient progress and adjust treatment plans as needed. Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation. Travel throughout Davis and surrounding communities to provide high-quality in-home physical therapy care. Qualifications Active California Physical Therapist (PT) license required. Home health Physical Therapist experience preferred. 1+ year of Physical Therapist experience preferred. Master's or Doctorate degree in Physical Therapy preferred. Current BLS or hands-on CPR certification preferred. Valid driver's license and reliable insured vehicle preferred. HCHB/Homecare Homebase experience preferred. OASIS experience preferred.
