Horizon House LPN PositionHorizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population, focusing on individual strength and choice.We are seeking qualified Licensed Practical Nurse (LPN) interested in joining our Residential Group Home team at our Sussex County, Delaware State location.Work Schedule: This full-time position is scheduled for 40 hours per week with 8-hour shifts. Work schedules are based on program and operational needs. Candidates must be available to work one of the following schedules:Monday–Friday: 4:00 PM – 12:00 AMMonday–Thursday: 4:00 PM – 12:00 AM, and Friday: 12:00 PM – 8:00 PMSome of the responsibilities include the following:Assist in the development, implementation, evaluation, and revision of recovery plans in collaboration with the participants and assigned staff.Maintains a case load of assigned participants and completes assigned paperwork in a timely manner.Act as a liaison, consulting with community agencies and families to maintain coordination of care in the treatment process.Provides ongoing assessment of participants' mental health symptoms and responds to recovery plans.Educates and supervises participants in self-administration of medication.Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.Qualifications:Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.Knowledge of mental health systems and supports.Knowledge of substance abuse systems and supports.Good communication skills both oral and written.Ability to work independently.Possess a valid driver's license with an acceptable driving record.Strong organizational skills.Successful completion of the Child Abuse Registry and the Adult Abuse Registry background check with no adverse registry findings, and obtain service letters in accordance with 19Del Code Section 708 and 11 Del Code Sections 8563 and 8564.Scope:Frequent talking and listening.Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.Local travel required, including transporting participants.May be required to report to work during emergencies, including inclement weather.May require crisis intervention in emergency situations.Scheduling flexibility and possible on-call work are necessary.Horizon House offers competitive compensation and a comprehensive benefit package including medical and dental, (9) nine paid holidays and generous time off, tax shelter annuity, tuition reimbursement, retirement savings plan, opportunity for advancement and more.To be considered as an applicant for this position, please complete the pre-employment application by going to House, Inc. is an Equal Opportunity Employer#ZREBEqual Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
