Responsibilities Provide skilled nursing care in the home setting Monitor patient status and report changes to the clinical team Administer medications and treatments as prescribed Assist with activities of daily living and personal care needs, as applicable Perform wound care and other skilled nursing interventions Educate patients and families on care plans and treatments Maintain accurate and timely documentation Collaborate with physicians, caregivers, and interdisciplinary healthcare teams

Qualifications Current Pennsylvania Licensed Practical Nurse (LPN) License Current BLS/CPR Certification Previous nursing experience preferred Homecare, long-term care, rehabilitation, or pediatric experience welcomed Strong communication and assessment skills Ability to work independently in a homecare environment Reliable transportation and commitment to scheduled shifts
