• Duration: 13 weeks
• Shifts: Monday-Friday, 8:00 AM - 4:30 PM
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours/week
• Estimated Weekly Pay: $2,030 - $2,190
• Mileage: Reimbursed between visits
• Openings: 2 Travel Physical Therapist positions

Qualifications & Requirements

• Bachelor's degree or higher in Physical Therapy (Doctor of Physical Therapy preferred)
• Current Physical Therapist license in Washington or PT compact license with privilege to practice in WA
• At least 1 year of experience as a Physical Therapist (home health experience preferred)
• Current CPR certification
• Valid driver's license, current vehicle insurance, and reliable vehicle for home visits
• Ability to wear solid color scrubs (no prints)
• HomeCare HomeBase experience is a plus

Key Responsibilities

• Evaluate patients' functional status and physical therapy needs and develop individualized home health care plans in collaboration with physicians and the interdisciplinary team
• Provide home health physical therapy interventions to relieve pain, restore function, and maximize patient independence using evidence-based techniques
• Document progress notes, evaluations, and required reports promptly and in accordance with agency policy and state regulations
• Monitor and report patient responses to treatment and changes in condition to nurse supervisors and physicians
• Educate patients, families, and caregivers on home exercise programs, safety, fall prevention, and activity progression
• Supervise home health aides and PTAs as permitted by Washington state regulations and agency policy
• Participate in staff conferences, in-service education, and quality improvement initiatives
• Maintain current clinical knowledge, licensure, and CEUs required for Washington physical therapist jobs
• Provide care across the service area with flexibility to meet patient scheduling needs
