Work Environment

The work setting is onsite at our modern rehabilitation facility in Oakland, which is designed to foster a healing atmosphere. Scheduled work hours are from Monday to Friday, 2 PM to 8 PM, allowing for a balanced approach to work and personal life.

Key Responsibilities

• Conduct comprehensive patient assessments to evaluate their therapeutic, rehabilitative, and functional status, ensuring each care plan is tailored to their unique conditions.
• Develop and implement individualized care plans in collaboration with physicians and the patient care team, focusing on achieving measurable healthcare outcomes and enhancing the overall patient experience.
• Deliver skilled physical therapy interventions, including therapeutic exercises and manual therapy techniques, in accordance with the patient's treatment goals and the physician's directives.
• Instruct patients and their families on therapeutic exercises, equipment usage, and home care strategies, empowering them to take an active role in their recovery.
• Oversee and prioritize daily caseloads, ensuring effective coordination of patient appointments and timely communication with team members regarding ongoing patient care.
• Maintain accurate and up-to-date documentation of patient care activities, treatment interventions, and outcomes to ensure compliance with ethical standards and legal regulations.

Patient Population / Client Focus

The Physical Therapist at Vitalize will serve a diverse population ranging from pediatric to adult patients, addressing a wide spectrum of conditions, particularly within orthopedic care. This includes post-surgical rehabilitation and chronic pain management. Qualifications

• Must hold a valid license to practice as a Physical Therapist in California.
• A Doctorate in Physical Therapy (DPT) is required.
• Basic Cardiac Life Support (BCLS) certification is required within 30 days of employment.

Compensation and Benefits
