Clinical Services Full Time Best in Industry 3000 NJ 10 W, Morris Plains, NJ 07950 Job Description The group facilitator is responsible for planning, guiding, and managing group and family therapy to ensure that all objectives are met effectively. The facilitator’s duties also involve selecting appropriate methods and tools that will enable participants to achieve results, reach decisions, and accomplish goals during treatment. Key Responsibilities ✓ Coordinate and provide care that is safe, timely, effective, efficient, equitable and client-centered;
- Handle case assignments, draft case management service plan, review case progress, and determine closure of case management cases;
- Enter data into and manage an outcome-tracking database;
- Help clients achieve wellness and autonomy;
- Facilitate multiple care aspects (case coordination, information sharing, etc.);
- Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options;
- Develop effective working relations and cooperate with the medical and behavioral health teams throughout the entire case management process;
- Arrange abeyances with social service, health, and governmental agencies;
- Take the extra mile and interact with clients to keep track of their progress and ensure satisfaction;
- Record case information. Complete all necessary forms accurately and produce statistical reports;
- Assess and address motivational and psychosocial issues;
- Adhere to professional standards as outlined by protocols, rules, and regulations; and other duties assigned by supervisor(s).

- While performing this job, one typically spends time sitting, walking, standing, driving, listening, speaking, typing, reading, and writing. In addition, an employee may be asked to lift or carry up to 20 lbs.
