Community Health Advocate: Outreach & Harm Reduction
Trillium Health
TRILLIUM HEALTH INC. is seeking a Community Health Specialist in Rochester, NY, to deliver harm reduction, HIV/HCV testing, outreach, and service navigation within a multidisciplinary health team.
The role focuses on trauma-informed care and health equity across the Greater Rochester and Finger Lakes regions. You will engage clients through outreach and education, link to care, support prevention services (PrEP/PEP), and assist with insurance enrollment and resource access.
