Social Worker (Geriatrics And Extended Care)

This Social Worker (Geriatrics and Extended Care) is located within Geriatrics and Extended Care service at the Providence VA Medical Center. The position is full-time at 40 hours per week. The Geriatrics and Extended Care Social Worker provides Social Work support to the programs within the GEC service line, including Hospital in Home, Community Nursing Home, Home Maker/Home Health Aide, Adult Day Health Care, Home Based Primary Care, and Palliative/Hospice.

Major duties and responsibilities include, but are not limited to:

• Visits the veteran as required and provides consultation and guidance to nursing home staff.
• Assists with making community referrals, coordinates specialty care within the VA and works in conjunction with the facility social worker in discharge planning when appropriate.
• Serves as a liaison between VA and the CNH program concerning administrative and veteran-specific clinical issues and reports all sentinel events within a 24hr period.
• Assists in providing informal and formal education to VA staff as well as community home staff in policy and changes.
• Monitors quality management measures to assure compliance with designated performance.
• Maintains contact with state regulatory and state ombudsman programs to monitor quality of care in CNH facilities and participate in annual meeting.
• Participates in conducting annual reviews of the CNH as part of interdisciplinary team.
• Provides Home Health Aide staff coverage.
• Conducts follow-up phone calls to veterans who have recently initiated services.
• Provides coverage during absence for the CHN Coordinator as necessary and Serves as back up for SVH responsibilities.
• Assists in assessing the needs of veterans assigned and documents the social support services provided.
• Evaluates and continually reevaluates the veteran's current psychosocial situation, to include financial, residential status, and coordinates any equipment or other needs.
• Provides high-risk psychosocial assessment in crisis interventions.
• Evaluates and provides care or case management appropriate to the assessed level of care.
• Supports other elements of the organization and performs other duties as assigned.

Work Schedule: Monday - Friday, 8:00am-4:30pm

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
