Licensed Physical Therapist

We are seeking a compassionate and skilled licensed physical therapist to join our team in the Midwest region. In this role, you will evaluate, examine, treat, and continuously assess patients to develop personalized treatment plans. You will maintain clear communication with patients, their families, caregivers, and healthcare providers, ensuring a collaborative approach to care.

Key Responsibilities:

• Conduct thorough patient evaluations and develop evidence-based treatment plans.
• Document evaluations, progress notes, discharge summaries, and other required reports promptly and accurately.
• Collaborate with rehabilitation team members to coordinate comprehensive care services.
• Create effective home exercise programs to support patient progress outside the clinical setting.
• Supervise physical therapy assistants and support staff, ensuring compliance with federal and state regulations.
• Assist with equipment maintenance and manage supply inventory.
• Participate in ongoing training and present educational sessions as needed.

Education and Certification:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy.
• Current physical therapy licensure and board certification as required by state regulations.
• Up-to-date Basic Life Support (BLS) certification from the American Heart Association.
• Completion of Mandatory Reporter training within six months of hire.

• Strong interpersonal skills with a service-oriented approach toward patients, families, and colleagues.
• Ability to develop and modify treatment plans based on patient progress.
• Commitment to adhering to all federal, state, and professional licensure standards.
