Compassionate Hospice Referral & Admissions Advocate

EverHeart Hospice is seeking a compassionate Referral Advocate to connect patients, families, and referral sources with our hospice and palliative care services. The role educates on care options, supports admissions, and builds relationships across the service area.

You will travel within Central and Western Ohio, assist with pre-admission paperwork, and collaborate with the Admissions Team to ensure timely care for eligible patients. Strong communication and organizational skills are essential.
