Clinical Pharmacist, Ambulatory Care & Population Health
HHM Health
HHM Health in Dallas, TX is seeking a Clinical Pharmacist to lead medication therapy optimization across ambulatory services. You will collaborate with providers to implement CPA-driven care, manage chronic diseases, and support HIV prevention efforts while focusing on underserved populations.
The role emphasizes patient education, safety, and 340B program involvement, with additional duties in quality improvement and data reporting. Texas licensure and PharmD are required; BLS is preferred.
