Youth Care Navigator: Wraparound & Family Support

Lake County, IL

Lake County Health Department and Community Health Center is seeking a Care Coordinator to support youth in the Pathways to Success program. The role involves home-based service delivery with travel across Lake County for meetings with youth and families, coordinating care and ensuring documentation is up to date.

Responsibilities include managing caseloads, creating team plans with families, and facilitating access to services while maintaining IM+CANS certification.
