Registered Nurse, Case Manager (Day Shift)
ARMStaffing is looking for a Registered Nurse, Case Manager in Portland, Maine.
Location: Portland, ME
Assignment Duration: This is a 13-week contract position.
Schedule/Shift: Monday through Friday, with every third weekend and holiday required. The shift is from 8:00 AM to 4:30 PM.
Job Description: The Registered Nurse Case Manager will be responsible for discharge planning and care coordination for patients in a high-acuity and complex care setting. The role involves working closely with patients, families, and healthcare teams to facilitate optimal patient outcomes.
Responsibilities:
- Conduct assessments and develop discharge plans tailored to individual patient needs.
- Coordinate care services to ensure smooth transitions from the hospital to home or other settings.
- Collaborate with healthcare teams to optimize patient discharge processes.
- Address and resolve potential barriers to discharge and follow-up care.
- Utilize electronic medical record (EMR) systems including EPIC and Wellsky for documentation.
Required Experience: Minimum of 5 years of hospital case management experience and 5 years of acute adult care nursing experience is required.
Certifications/Licensure: Active Registered Nurse (RN) license is required.
