The outpatient clinic, located in Century City / Los Angeles, CA, focuses on coordinated, multidisciplinary care, providing PT, OT, and SLP services with an emphasis on smooth transition through the continuum of care. Responsibilities Duties Completes a comprehensive evaluation of assigned patients and uses clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients toward discharge within an established length of stay. Collaborates with the interdisciplinary team to address all barriers to discharge, including timely equipment trialing and ordering, patient and family training, and planning for continuing services. Demonstrates flexibility in caseload management, including accepting coverage and ensuring the plan of care is communicated, implemented, and progressed throughout the length of stay. Manages individual, concurrent, and group treatment sessions based on patient care needs. Completes medical record documentation in a clear, concise, accurate and timely manner, including daily documentation, goal writing, FIM entry, letters of medical necessity, discharge documentation, and charge entry. Creates an individualized professional development plan with clinical management, pursuing ongoing education and involvement in professional activities. Participates in patient and professional advocacy efforts, including lobbying activities, health fairs, community events, professional memberships, and fundraising or charitable activities. Adheres to the discipline specific State Practice Act and other regulatory agencies, maintains professional licensure in good standing, and completes all state mandated continuing education requirements. Understands the organization’s performance improvement methodology and follows through with action plans, offering feedback through staff meetings, focus groups, or direct supervisor.
