The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers. Job Requirements In addition to bringing humankindness to the workplace each day, qualified candidates will need the following: Required Qualifications Three years of experience required, with one year in specialty Bachelor Degree Nursing Registered Nurse in State of Employment Basic Life Support through the American Heart Association Strong psychosocial assessment and clinical skills Preferred Qualifications Nursing Professional Cert through American Nurse Credentialing Cente Physical Requirements Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly Where You'll Work Located in Lakewood, Colorado, St. Anthony Hospital is a Level I Trauma Center where we provide a full range of medical specialties and health care services to Denver and the surrounding region through our state-of-the-art medical campus and home base for Flight For Life Colorado.
