Compassionate Hospice RN Case Manager

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate, in-home nursing care and to lead patients and families through their hospice journey. You will assess, plan, implement, and document care aligned with goals of care, coordinating with an interdisciplinary team and ensuring comfort and dignity at every step. This full-time role requires an active RN license and hospice experience; you will educate and support families, manage documentation, and participate in #J-18808-Ljbffr
