Home Health RN Case Manager

Visiting Nurse Association of Wisconsin, Inc., affiliated with Aurora Health Care, is seeking a compassionate Home Health RN Case Manager to serve patients in the Oshkosh and Fox Cities market. In this field-based role, you will provide skilled nursing care in patients’ homes, assess health needs, develop and update individualized care plans, monitor recovery and chronic conditions, and educate patients and caregivers. You will coordinate closely with physicians, families, and an interprofessional team, maintaining accurate clinical documentation and supporting safe, patient-centered care.
