The team leader provides clinical and administrative supervision of the team and practices as a clinician on the team. Functions: Clinical Providing and coordinating the assessment and reassessment of the recipient’s clinical needs Convening the Child and Family Team for person-centered planning Completing the initial development and ongoing revision of the Person Centered Plan and ensuring its implementation Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process Providing and coordinating treatment for the youth or other family members with other licensed professionals and Child and Family Team members Ensuring linkage to the most clinically appropriate and effective services Planning and facilitating consumer transitions to other services and/or local agencies in an efficient manner Completing and submitting all appropriate ITR and other required discharge documentation to the appropriate state, LME, or state contractor Submitting completed discharge date to the Medical Records Specialist for processing and filing Support Services Assist the youth and family served by providing support and guidance in all areas of functional domains including adaptive, communication, psychosocial, problem solving, and behavior management. Provides a time-limited intensive family preservation intervention to persons served in order to stabilize the living arrangement, promote reunitification or prevent the utilization of out-of-home therapeutic resources for youth Participates in a team approach to address the identified needs of children and adolescents who are transitioning from out of home placements or who are at risk of out-of-home placement in order to remain stable in their community Assists youth served in defusing any current crisis by evaluating its nature and providing intervention to reduce the likelihood of a recurrence Ensures linkage to needed community services and resources by identifying the need of the youth served and researching any available programs in their community Provides self help and living skills training to youth served as well as providing parenting skills training to help the family build skills for coping with the youth’s disorder Monitors and manages the presenting psychiatric and/or addiction symptoms of the individual served including referrals when needed and assists individual in recognizing early warning signs that a crisis may be near Works with caregivers in the implementation of home-based behavioral supports including but not limited to: crisis management intensive case management individual and/or family therapy substance abuse intervention skills training other rehabilitative supports Provides a person-centered focus on the youth served in order to facilitate a nurturing environment which will prevent the need for an out-of-home or more restrictive service Assists the family of the youth served in maximizing independence for the family and ensures a full partnership in treatment with minimal intrusion. Provides intensive services in the beginning of treatment that can successfully decrease over time as the youth and family’s coping skills develop Participating in the person-centered planning process Providing direct home-based behavioral health services and supports, including therapy for the family, as indicated in the Person Centered Plan Providing indirect services for the family as indicated in the Person Centered Plan Monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person Centered Plan Consulting identified non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process Support the persons served in understanding and exercising their rights. Provide support services in a manner that is safe, therapeutic, and free from abuse, neglect, and exploitation. Ensure that all information resulting from health related appointments is documented and communicated appropriately Attend and participate in treatment team meetings, audits, and committees as requested. Case Management functions as assigned. Administrative Ensure that services rendered to persons served are billable via management of cases and/or providing direct services Assemble a team of appropriately trained and qualified staff to ensure continuity of care to individuals served Train support team staff on Agency philosophy of service delivery, as related to applicable service definitions, statues and rules Provide supervision of support team staff Conduct performance evaluations for support staff team and consult with the Clinical Director regarding results and recommendations In concert with the Clinical Director, take appropriate disciplinary actions consistent with Agency policies and procedures Represent Agency at local management entity and other necessary meetings, as related to the care of persons’ being served Submit all completed documentation to the Medical Records Specialist for filing, billing, and payroll purposes Quality Assurance and Improvement Assure consistent provision of quality services to consumers Assure the completeness of the medical record. Monitor the implementation of the PCP to assure that services are being provided in a manner that is professional, effective, and consistent with governing rules, statutes, service definitions, and the PCP Assure that case management, supervision, and service notes are being completed in a manner consistent with Federal, State, and Agency, statutes, rules, and policies and procedures, respectively Provide quality review and sign-off of service notes, checking them against goals as delineated in PCPs Provide training to support team for remediation of documentation issues, as needed Provide quality review and sign-off on mileage and timesheets of support staff team Review, sign, and submit supervised staff’s service documentation to the Medical Records Coordinator for billing verification, filing, and submission to the Home Office, as applicable Conduct routine follow-up with service recipients, family members, and guardians regarding their satisfaction with service delivery Participate in peer review activities of Agency offices as requested Communicate ideas on improving services to Local Administrator Assure that staff’s vehicle(s) are safe and in good working order Communication Maintain consistent communication with the Clinical Director regarding documentation, authorizations, team and individual support staff performance, consumer progress toward goals, and consumer satisfaction Maintain regular contact with persons served, their treatment teams, guardian and significant others in persons’ lives Documentation Service notes Complete all service documentation including but not limited to the following: service notes, incident reports, medication administration, etc. on the appropriate form and in a thorough and timely manner, as required by State, Federal, and Agency rules, statutes, policy and procedure, and Agency service operation manuals. Submit service notes to the appropriate supervisor on a weekly basis Time sheets Accurately and legibly complete timesheets, reflecting the amount of time spent in service provision Submit timesheets to the appropriate Team Leader on a weekly basis NOTE : Personal timesheets, including supporting service notes must be submitted by 10:00am each Monday . [Failure to submit this information will result in a decrease in the hourly rate to minimum wage.] Qualifications: Education/Experience The minimum requirement for this position is: Master’s level licensed professional in one of the following licensed discipline categories: NC Licensed Clinical Social Worker NC Licensed Psychologist (Doctorate Level) NC Licensed Psychological Associate NC Licensed Professional Counselor NC Licensed Marriage and Family Therapist Mastered Degree NC Licensed Nurse – advanced practice only (NP, PNPs) NC Licensed Clinical Addiction Specialist Certified Clinical Supervisor Board Eligible staff may serve as Team Leader. However, full licensure must be attained within 30 months of hire. Must have knowledge and one year’s documented experience with the age and population served Preferred Experience One or more years of supervisory or case management experience Training Must successfully complete all state required trainings pursuant to the support service delivered Must successfully complete all required Agency trainings and in-services Additional Requirements Valid Driver’s License, reliable automobile with proof of current liability insurance, acceptable driving record, current vehicle registration and inspection, current TB test, and a clean criminal record. Basic mathematical skills Ability to read, write and understand the language of the person served and read and write in English. Ability to work as a team member Reliable transportation Productivity Expectation: Provide support services as scheduled by supervisor, maintaining a 50% productivity rate This position requires a minimum number of units expected to be performed of 20 units/week, 10 being face-to-face sessions and 10 coordination of care (COC). This may vary on a week-to-week basis due to client need (a client may require more than 1 face-to-face or COC session on top of the minimum requirements). Maintain therapeutic boundaries with consumers, families, and guardians Be familiar with HIPPA as related to confidentiality of information specific to persons served Have an understanding of Client Rights Have an understanding of person served abuse, neglect, and exploitation rules Develop and consistently demonstrate an understanding to the persons served, their likes, dislikes, behaviors, desired life outcomes, visions for the future, and their PCPs Have a basic understanding of mental health and related symptomology Have or develop an understanding of the medications that the persons served may be taking, their purpose and side effects Have or develop a working knowledge of therapeutic interventions to assist persons served in effectively monitoring and dealing with their emotions and behaviors Consistently practice appropriate medication administration and documentation Consistently practice appropriate service records documentation, pursuant to State, Federal, and Agency Policies and Procedures Provide support services as scheduled, showing up on-time and working on person-centered goals for the duration of the time allotted Serve as a valued member of a person-focused team to support the individuals we serve Establish and maintain effective working relationships with colleagues and persons served Express ideas clearly, both verbally and written Ability to plan and execute work effectively Be aware of expiration dates and maintain current training and recertification requirements Attend and participate in all team meetings, committees, and audits as requested Accuracy Required In Work And Consequence Of Error: The Intensive In-Home Team Leader is responsible for insuring systems are in place to address the on-going facilitation of services to each consumer, as well as oversees the collective services of his/her caseload , which has the potential to severely affect the health and safety of consumers and staff. This position has a fiduciary responsibility to the Agency including but not limited to billing, in which errors could result in significant funding infractions for the agency Supervision and Training Provided To Employee : The local office will provide initial training. On-going training and supervision is provided on an on-going basis by the supervisor. Physical Effort: A variety of physical activities are necessary for this position and depending upon the needs of the consumers of the agency. Travel is required within the community. From time to time out-of-town travel may be required. There will also be interaction with consumers served at each local office.
