Quail Run Hospital is a 115-bed, acute care psychiatric hospital located in Phoenix, Arizona. Quail Run features individual units for adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 10,000 patients receive care from our compassionate health care team each year at Quail Run Behavioral Health.

The Intake And Assessment Nurse is a registered professional nurse who is responsible for assessing the medical and behavioral needs of the patient. The Intake And Assessment Nurse assists in the process of responding to referrals, schedules patient assessments/appointments, triages patients, and maintains communication with referral sources, families, patients and unit staff. May be assigned Team Lead role at the discretion of the Director of Admissions.

Job Duties:

• Answers telephone within 3 rings using the appropriate greeting and attempts to obtain all pertinent clinical, demographic and insurance information from the caller to determine caller's needs.
• Keeps aware of calls waiting in the queue which require immediate attention.
• Completes a thorough and appropriate medical evaluation within the scope to support the admission process, ensuring all required clinical information is accurately gathered, documented, and communicated to facility in a safe and timely manner.
• Conducts an initial clinical assessment on all individuals presenting for admission, which includes treatment and medication trial history, family dynamics, etc., and establishes an initial diagnosis for patients who are admitted.
• Notifies unit staff of pending admission and provides pertinent demographic information.
• Schedules assessments, psychiatric evaluations and admissions with potential patient/referral source within 24 hours.
• Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for hospital stay.

Qualifications:

Current license to practice nursing in Arizona or compact privileges (out of state compact license: must apply for an AZ license within 60 days of hire). Prior intake, assessment, admission, and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting strongly preferred. Handle With Care and Basic Life Support (BLS) for Providers required. Training provided at the facility. Level 1 Arizona Fingerprint Clearance card or be able to obtain within 30 days of hire.
