Licensed Practical Nurse

LPNIn keeping with standards recognized by both NCQA Medical Home Recognition and JACHO, SWLA has created teams of professionals to care for the clinical needs of patients who choose SWLA as their primary care provider. The team consists of physicians, mid-level practitioners and the nursing support which is described in this job description. Given the nature of the duties required by the nursing support personnel, SWLA will recruit and hire applicable personnel in the categories of Medical Assistant, Certified and non-certified, and Licensed Practical Nurses. Salary will be adjusted for education, licensing acquisition and experience.Education, Training and Experience:High School Graduate. Successful completion of Practical Nursing course from an accredited Technical School.Successful completion of all requirements of Louisiana Board of Practical Nursing must be met if applicable. Current State Licensure if applicable.Current BLS/IV Certification. CEU'S or equivalent contact hours for re-licensure or re-certification, if applicable. Must successfully complete a competency exam.Must maintain a professional attitude under pressure.Employ correct grammatical English and write legibly in order to complete appropriate paper work.Present a professional image and be able to maintain the highest level of quality patient and community relations in representing the foundation.Job Responsibilities:Participates as an integral part of the multidisciplinary care team.Responsible to greet patients and ensure that they do not spend excess waiting time in the exam room.Responsible for the collection of necessary information concerning the patient in order to assist the physician in providing medical care.
