Hospice Care Transition and Referral Coordinator
St. Croix Hospice
St. Croix Hospice in Michigan seeks a Care Transition Coordinator to build and maintain relationships with referral sources and communities, while educating patients and families on hospice care to support informed choices. The role also assists intake to ensure timely referrals and coordinates marketing activities with the Director of Patient Services.
